Q) Which treatment for erectile dysfunction is preferred the most?

A) Erectile dysfunction is an extremely common concern in men and their partners with prevalence (between 13 to 28 per cent) in those aged 40 to 80.

It is defined as the recurrent inability of a man to attain and/or maintain an erection sufficient for sexual activity. This has the potential to affect both partners psychologically, causing emotional distress and a loss of self-esteem. 

As such, pharmaceutical companies have introduced an array of options to help correct this situation. Not surprisingly, the oral drugs known as the PDE-5 inhibitors (Cialis - tadalafil, Viagra - sildenafil, and Levitra - vardenafil) are far preferred to the use of injections (Caverject) or urethral suppositories (Muse) although they don’t necessarily work any better. 

A number of studies have come out comparing the three tablets over the years and the results have been consistent regardless of differences in the patients compared, time-frames studied or doses used.

Depending on the study, 52 to 65 per cent of patients preferred Cialis versus 12 to 20 per cent for Levitra and eight to 30 per cent for Viagra. Interestingly, the numbers were roughly the same when the partners were queried as to which drug they preferred the man to be taking. 

The main reason stated for choosing Cialis was its much longer duration of action as it can work for up to 36 hours after being swallowed giving couples the opportunity for more spontaneity, whereas its two competitors tend to be effective for only four to five hours. 

 

Another reason stated in favour of Cialis was that it is not affected by the presence or absence of food, while both Viagra and Levitra can have their onset of action slowed and the extent of their absorption lowered in the presence of a high-fat meal. 

Generally, the number of complaints regarding side effects was similar, and all three drugs compared fairly evenly when patients were questioned about erection hardness or intercourse satisfaction. As well, the drugs are priced roughly the same. 

While Cialis clearly seems to be the choice for most patients, it is important to remember that a significant number of men do prefer Viagra or Levitra. So, if you are not happy with the results with any of these medications, changing to one of the others may result in a much better experience.
